Family Forum Presentation: Athlete Eligibility; SOILL Consent and Medical Forms 101

Special Olympics Illinois recently introduced a new Consent Form and Medical Form to replace the existing Application for Participation (Med App). This latest offering from SOILL Family Forums aims to discuss this new initiative so parents, caregivers, and agencies are armed with the information they need to complete these forms and maintain athlete eligibility.

Our new Medical Form will replace all currently available Medical Applications and is the only form that will be accepted beginning Aug. 1, 2021. Make sure you know about all the changes and what to do by registering to attend.

The forum will be led by Region H Director, Lynde Pavich, on Wednesday, July 21. Join Lynde as she discusses these forms, details the new process, and answers frequently asked questions. The session will run from 5:30 PM to 6:15 PM CT.
